Adult & Minor Medical Release Forms:

  1. Please print out the apporpriate form, fill out, and fax to 760.929.0169.
  2. Download Adult Relase Form
  3. Download Minor Release Form

PROCEDURES for LEADERS of CAMPING, BACKPACKING, and HIKING TRIPS:

  1. Secure your permit first & foremost.
  2. Send a copy of permit to Sheri for reimbursement c/o NCCC, 7220 Avenida Encinas, Carlsbad, Ca 92011 Attn: Sheri.
  3. Download/print the blank form for camping, backpacking & hikes.
  4. Fill in the blanks thoroughly and return to Sheri at least 6 weeks out from the hike date for advertising purposes. E-mail Sheri@northcoastcalvary.org  (Call ext. 124 if questions).
  5. Please stop by the office prior to the hike or trip and pick up the Hiking box and a First Aid kit from Orville or Sheri. (Not on Fridays)
  6. Gather information to be distributed at the Orientation meeting. Sheri will be glad to assist.
  7. If you are a Day Hike Leader, please download/print the blank form for day hikes, fill in the blanks and email to Sheri in the same time frame.
  8. After the hike or the camping trip, let Sheri know what went right, what went wrong and what to change for next time.
